TPI Drop/Add Form

Please download the drop/add form to report any drop or addition for classes during the given term. This form will need to be completed in its entirety and submitted to The Potter’s Institute office in the following manner:

1. In person, by coming to the office during normal business hours – Tuesday thru Friday, 10am – 4:30pm

2. By faxing to 214-333-6512 or

3. Forward to the TPI Representative that will be serving at the TPI table in the church lobby on certain Sundays or

4. Forward to the TPI Representative at the facility where the class is being held.
